Pitch-Tracking Risk Factors and Warning Signs for Shoulder Capsulolabral Injuries in Major League Baseball Pitchers

A progressive velocity decline in the final outings before shoulder surgery is a promising but unproven Statcast warning sign, not a screening tool yet.

Retrospective case-control study, n=27 MLB pitchers who underwent arthroscopic capsulolabral repair, matched 1:2 to 54 uninjured controls using Statcast pitch-tracking data (161,979 pitches over 2 preinjury seasons).

Cases threw harder and spun the ball more at baseline (+1.46 to +1.57 mph on fastballs/sinkers, P=.006-.009; spin rate β=+0.32 SD, P=.02) with less pitch-to-pitch variability in arm angle (β=-0.58 SD, P=.005) and release speed (β=-0.37 SD, P=.007). In the injury season, cases lost more fastball (-0.68 mph, P=.009) and breaking-ball velocity (-1.07 mph, P=.003) than controls, and in the final 5 outings before injured-list placement showed a progressive linear velocity decline (-0.20 mph/outing fastball, -0.30 mph/outing breaking ball, both P<.001).

Studiedesign

Retrospective, matched case-control study (Level of evidence 3), 27 surgical cases matched to 54 uninjured controls, using publicly available Statcast pitch-tracking data rather than clinical exam or lab-based biomechanics.

Metode

Matching on age, role, handedness, season and pitch count limits some confounding, and mixed-effects models with clustered standard errors are appropriate for repeated pitch-level data, but the design is retrospective and non-randomised, vulnerable to selection bias (who actually gets surgery versus managed conservatively is a team/clinical decision) and reverse causation (harder throwers may simply accrue more shoulder stress over time regardless of any distinct 'signature'). Six variables were tested across 7 pitch types in 3 separate analyses without a clearly stated multiple-comparison correction, so some of the borderline p-values (.02.009) could be chance. 'Kinematic' here means Statcast release-point metrics, not true 3D shoulder biomechanics, so any mechanistic link to capsulolabral strain is inferred rather than measured.

Vurderingen

The baseline velocity/spin differences are statistically significant but small in absolute terms (about 1.5 mph, roughly 1.5-2% of typical fastball speed), unlikely to be meaningfully distinguishable pitch-to-pitch by a coach or clinician. The reduced-variability and progressive pre-injury velocity-decline findings are more compelling, larger standardised effects with a consistent linear trend, but a case-control design can only show association with a pitcher who ultimately needed surgery, not prove a causal preinjury mechanism.

There is no prospective test of whether monitoring these signals in real time, such as a velocity-decline alert threshold, actually catches at-risk pitchers early or changes outcomes; as it stands this is a retrospective pattern description, not a validated screening tool.

This extends the established pitching biomechanics and workload literature (kinetic and pitch-count injury research from groups like ASMI) into the newer era of Statcast-based population surveillance, joining a small but growing body of Statcast injury-epidemiology studies that has so far focused mostly on UCL/elbow injury; there is no established landmark specifically linking Statcast release metrics to shoulder capsulolabral injury yet.

For MLB/MiLB or elite-level medical and performance staff with pitch-tracking data, a progressive drop in fastball or breaking-ball velocity across consecutive outings (roughly 0.2-0.3 mph per outing) is worth using as a trigger for closer shoulder assessment, not as a stand-alone red flag on its own; this is a retrospective association in players who already needed surgery, not a validated alert threshold. It's also elite-pitcher-specific data: don't extrapolate the velocity ranges or thresholds to amateur or youth throwers who aren't tracked this way. Clinicians without this level of tracking infrastructure won't find this changes Monday's practice.

In practice

This applies narrowly: MLB/MiLB-calibre pitchers (or any program with Statcast-equivalent tracking), not amateur or youth throwers, where this data doesn't exist and velocity/workload ranges are different. In the clinic, if performance staff flag a pitcher showing a 3-5 outing pattern of progressive velocity drop (roughly 0.2-0.3 mph/outing) on fastball or breaking ball, don't wait for a pain report, bring them in for ER/IR rotation ROM and a posterior shoulder endurance test to catch a GIRD or cuff-fatigue pattern before it progresses to a labral tear, and baseline these on rostered throwers preseason so a later change has something to compare against. On the S&C floor, treat a pitcher who's quietly throwing harder and spinning more with unusually low outing-to-outing variability (the baseline profile of the surgical cases here) as a mechanical-efficiency flag worth a technical check-in, not just a performance win. Caveat: this is a retrospective signal from players who already needed surgery, so a velocity dip on its own is a trigger to look closer, not a reason to shut a healthy-feeling pitcher down.
