The Epidemiology of Catastrophic Head and Neck Injuries in High School and College Non-Football Sports: A 4-Decade Review

Orthopaedic journal of sports medicine · 2026

Catastrophic head/neck injuries in non-football sports have fallen roughly 10-fold since the 1980s, but high school gymnasts and hockey players remain the highest-risk group today.

Lo studio

Descriptive epidemiological (registry) study, n=470 catastrophic head/neck injury events across 17 non-football high school and collegiate sports, 1982-2022.

Cosa hanno trovato

CHNI rates were markedly higher in males than females (IR 0.48 vs 0.12 per 100,000 athlete-seasons; IRR 0.25, 95% CI 0.18-0.33, p<.001) and in high school than college athletes (IR 0.53 vs 0.11; IRR 0.20, 95% CI 0.15-0.27, p<.001). Gymnastics (IR 1.66) and ice hockey (IR 1.52) had the highest rates, and overall rates fell roughly 10-fold from the 1982-1992 decade (IR 0.59) to 2012-2022 (IR 0.06). 12.6% of events were fatal and 38.7% permanently disabling.

La valutazione

This is passive surveillance registry data (NCCSIR), not a controlled study, so it can only describe association, not cause. The headline 'decline over decades' is statistically robust but highly vulnerable to ascertainment bias — reporting completeness, denominator (athlete-exposure) accuracy, and case-finding methods almost certainly changed substantially between 1982 and 2022, and the paper can't fully separate a true risk reduction from better safety equipment/rules versus a true reduction in under-reporting historically. Rare-event counts (e.g., single-decade IRR of 8.93, CI 2.23-35.68) reflect genuine statistical uncertainty, and denominator data for participation was 'available' rather than complete, which limits precision of IRs by sport and era.

Il limite

Whether the apparent decline reflects real risk reduction (rule changes, medical coverage, equipment) or improving surveillance/reporting completeness over 40 years is unresolved — the authors cannot rule out ascertainment bias driving the historical rate gradient.

Studio di riferimento

This extends the NCCSIR's long-running catastrophic injury surveillance program, historically dominated by football-focused reports (e.g., Boden and colleagues' annual football catastrophic injury series); this paper fills a genuine gap by isolating non-football sports specifically.

Cosa fare da lunedì

Doesn't change acute on-field management, but supports directing medical coverage, emergency action planning, and spine-injury preparedness resources preferentially toward high school-level gymnastics and ice hockey programs, where rates and severity are highest and medical staffing is often thinnest.
