Chronic plantar heel pain severity is associated with lipid biomarkers

The paper

Cross-sectional case-control study, n=197 adults with chronic plantar heel pain and n=100 age/sex-matched controls.

What they found

Total cholesterol (β=5.0 points/mmol/L), non-HDL cholesterol (β=5.1), LDL cholesterol (β=4.9), and TC/HDL ratio (β=4.2 per unit) were positively associated with pain severity on a 100mm VAS among cases, but glucose, triglycerides and HDL were not; no lipid biomarker was associated with CPHP presence (case vs control) or pain duration, aside from a waist-circumference-by-LDL interaction on duration (β=0.02).

The appraisal

Reasonable sample size with matched controls and adjustment for age, sex and BMI, plus sensitivity analysis for lipid-lowering medication, but this is cross-sectional so temporality and causation cannot be established — worse lipids could just as easily be a consequence of reduced activity from chronic pain as a cause of it. The effect sizes are small (roughly 4-5mm on a 100mm VAS per mmol/L of cholesterol, a modest biological range), so even if statistically robust these are unlikely to be individually meaningful; no confidence intervals or p-values are given in the abstract, making it hard to judge precision. Residual confounding from physical activity, diet, and disease chronicity is a real concern, and the biomarkers weren't associated with having CPHP at all — only with severity among those already affected.

The gap

Whether the lipid-severity association reflects a causal metabolic contribution to nociception or is simply a marker of deconditioning/inactivity in people with more severe, longer-standing symptoms — only a longitudinal or interventional design (e.g., does lipid-lowering treatment change pain severity) could resolve this.

Landmark context

This extends the metabolic-syndrome/tendinopathy hypothesis (well established for Achilles tendinopathy, where hypercholesterolemia and xanthomas are classically linked to tendon pathology) to plantar heel pain, and sits alongside the broader plantar fasciitis literature linking obesity and BMI to risk — but is one of the first to look specifically at lipid subtypes rather than just BMI.

What to do Monday

No, this shouldn't change Monday's clinical management — clinicians shouldn't be ordering lipid panels or treating cholesterol as part of routine CPHP care based on this. At most, it's a reminder that patients with severe, longstanding heel pain may carry unaddressed cardiometabolic risk worth general medical follow-up.
